We will provide you with a Wholesale Application Form. You must submit a completed and acknowledged Wholesale Application Form as well as the following:

• VillageHouse Importers Wholesale Application Form
• Current business license or state registration (Retail Sales and Tax Certificate)
• Online resellers must own and operate a brick-n-mortar store.
• If you are requesting net 30 payment terms, provide 4 vendor credit references and bank reference(s).
• Credit Card payers must submit the completed and signed Credit Card Authorization Form.
